Lifelong Links

The Lifelong Links team are dedicated to helping young people who are leaving care and would like to reconnect with people they have lost through their care journey.

Lifelong Links coordinators will use various ways to search for and find family members (known or unknown) and other adults (previous foster carers, teachers, neighbours for example) who care about the young person. Once the people that the young person wants to find have been found a commitment ceremony will be held where everyone can make a life-long plan of support with and for the young person. This will be incorporated into the young person's care plan and social workers will work with the young person and the people who are their network of support while transitioning into adulthood.
