You have a right to see the information we keep about you. If you apply, we will provide you with a copy of your social services records. All third party information will be removed.

If you would like to see a copy of your file, please speak to your PA who will help you complete the Access to Records Request.

Think about the decision very carefully. Files can be very difficult to read.

It can take some time to get a copy of your files, so please be patient.
